A new tool on the NHS Inform website allows people managing a long-term health condition to put together a personalised ‘online leaflet’ of health and care information.
Originally developed to support people with cancer, the 'Info for Me' tool can now be used by people with a range of conditions.
Users can navigate around the site and use the ‘Info for me’ button on each page to select and pull together the relevant information for them from different parts of the site into one area.
The gathered information can then be printed, saved and shared with others.

It is one of a number of new features on the redesigned NHS Inform website, a national NHS website for Scotland with information about illnesses and conditions, healthy living, tests and treatments, and health and social care services.
As well as ‘Info for Me' there's also a refreshed self-help guide which people can use to assess their own symptoms.
The tool was developed by NHS 24 and Macmillan Cancer Support after patients said they'd have managed better if they'd known more about their treatment.
Macmillan’s programme manager for Scotland, Gordon McLean, said: “More people are being diagnosed with and surviving cancer than ever before and we know too many of them aren’t getting all the information and support they need."
NHS 24's head of Health Information Services, Lynne Huckerby, said: "The personalisation tools such as 'Info for Me' means that NHS Inform can make a huge difference, because we know that when people are better informed they make better choices, or manage a condition better.”
She added: “[NHS Inform] is a national resource available 24/7 that should be a first port of call for anyone with a query about health and care."
